The following are useful points to be considered when withdrawing a patient from a benzodiazepine:

Symptoms

When taken long-term (more than four weeks), the patient can become dependent on the drug and may experience withdrawal symptoms when it is discontinued. This withdrawal experience is unique and symptoms vary according to individual. Common physical symptoms include: profuse sweating, headaches, nausea, dizziness, gastric disturbances, palpitations, chills, muscle pain, twitches, spasms and tremors. Psychological symptoms such as feelings of depersonalisation, derealisation, anxiety, panic attacks, nightmares and distorted perception are also common.

There are doctors who are very aware of this and are able to reassure their patients that the symptoms are indeed withdrawal-related and will disappear once withdrawal is over. Sadly, this is not always the case and many patients with no pre-existing psychological problems end up being misdiagnosed and treated for schizophrenia, bipolar and other mental health disorders.

Cold-Turkey

A patient should never be advised to discontinue taking a benzodiazepine abruptly. "Then stop taking it" was the reply of a well-intentioned doctor when I expressed my concern that the drug had lost its efficacy. Fortunately, I found information online which recommended a slow taper using diazepam (because of its longer elimination half-life) and was able to successfully wean off. It is surprising that many doctors still give this advice and our Helpline often receives frantic emails from people experiencing extremely distressing symptoms as a result. Quitting cold-turkey is dangerous and can cause serious problems including seizures and psychosis.

Tapering

The decision to withdraw should be the patient’s and she or he must be allowed to taper off the drug at a comfortable pace using the most appropriate weaning process. The more common methods are: substituting with diazepam, titration by crushing the tablet into a powder and mixing it with water, and the direct method where the dosage is very slowly reduced. Factors to be considered include personal circumstances, overall general health, the stressors in the patient’s life, stamina, support available and previous experience with drugs. It is most important that the patient feels in control of the process. Apart from the usual withdrawal challenges, being pressured into tapering too quickly can cause additional anxiety and hinder recovery.

Duration

The conflicting reports regarding the duration of withdrawal and whether or not protracted withdrawal exists poses one of the biggest problems for patients. Withdrawal can last as short as between 5 and 28 days for those with mild dependencies. However, there are many cases where symptoms persist for longer and these patients are told the withdrawal period has ended and the problems are "all in the head". Furthermore, as alternative diagnoses are queried additional emotional energy is expended awaiting diagnostic tests results which are usually negative. When every test is exhausted, the suggestion that the problems are psychogenic in origin and nothing to do with withdrawal is inevitably made. This misinformation does not augur well for the unfortunate patients who then become concerned about the implied possibility of psychological disorders only to find that the symptoms disappear once the protracted period ends.

Benzo-wise doctors will agree that while many people recover within a six to eighteen-month period, it is not uncommon for a percentage of patients to experience symptoms (often interspersed with windows of normality) for two to three years or longer in rare cases.

Pre-existing Anxiety Myth

Because many patients are prescribed benzodiazepines for anxiety-related issues, the consensus is usually that the post-withdrawal syndrome or any protracted symptoms are in fact due to a resurgence of the pre-existing anxiety. I was prescribed a benzodiazepine for a neuromuscular condition and had no history of anxiety, depression or any other psychological problem. The anxiety I experienced especially during acute withdrawal was inconceivable. I have also communicated with others who were prescribed benzos for medical problems and experienced intense organic fear and numerous anxiety-related symptoms. Pre-existing anxiety or not, a nervous system in a hyper-excitable state due to the down-regulation of GABA receptors can reduce the most grounded and stable person to literally a ‘quivering wreck’.

It is the responsibility of every doctor who prescribes a benzodiazepine to give the patient information on which the decision to take or not take the drug can be based. When treating patients for anxiety, insomnia or other related conditions, a doct 1ff8 or might understandably be hesitant and conclude that imparting too much information will only make matters worse. However, keeping patients ignorant of the addictive properties of a drug is not in their best interest and this is the reason for the ‘unpleasant surprise factor’ that presents in the form of withdrawal.

Diazepam has been known to affect chemicals in the brain, causing imbalances, resulting in anxiety, seizures, and muscle spasms in patients. Diazepam may not mix well with other drugs, especially those that may cause drowsiness. Some of the drugs that should not be mixed with Diazepam are anti- depressants, antihistamines, sedatives, pain relievers, or medications that work as a muscle relaxant.

Antacids will lessen the effects of Diazepam. If antacids are necessary for a patient it is recommended that they be taken about three hours apart from each other. It is important to discuss with your doctor what medications you are currently taking as there are other medications, not mentioned here, that might either react or decrease the effects of Diazepam.

Activities that should be avoided while taking Diazepam are driving cars or machinery. Diazepam can cause drowsiness. Alcohol should not be consumed while taking this medication. It is likely that the consumption of alcohol can cause increased dizziness and drowsiness, and the risks of having a seizure are greatly increased.

Diazepam is to be taken as directed by your doctor. It is important to take the correct dose, if a patient misses a does it is recommended that they skip that dose and take the next dose as usual. If you forget to take it, do not take it again if it is real close to the next time you are scheduled to take it. Skipping one does is not critical. Taking two doses too close together can cause a lot of problems.

In conclusion, please note that Diazepam is habit forming. I strongly encourage you to discuss with your doctor the length of time you will be taking this medication.

I’m an advanced Emotional Freedom Techniques (EFT) Practitioner as well as being a recovered involuntary benzodiazepine (benzo) addict. I came off benzos in 1988 and qualified in EFT in 2006. That’s quite an unusual combination. I wish EFT had been around when I was coming off. But now I’m qualified, it puts me in the unique position of being able to offer EFT tapping routines to people who are withdrawing from a place of personal experience. And that’s really good news because if there’s one thing you need when coming off benzos - it’s the advice and support of others who’ve been through it - ’survivors’.

My own withdrawal in 1988

There wasn’t much information available when I came off, certainly nothing like there is now - internet sites, books and organisations. Professor Heather Ashton, world authority on benzo addiction and withdrawal, hadn’t set up her withdrawal clinic in Newcastle or written her manual (see below). I devised my own withdrawal programme, putting together a variety of complementary therapies and emotional /psychological support techniques. The result is ‘Safe Benzo Withdrawal’, my double CD set (see below) which contains advice and information based on my own experience. There is also a guided relaxation which I wrote specifically for benzo withdrawal. The data CD contains equivalence tables for switching to Diazepam (valium), a slower acting benzo.

Withdrawal Symptoms vary

Not everyone has a tough time coming off. We’re all different and there are many factors affecting the type of withdrawal i.e. dosage, type of medication, time you’ve been taking it etc. Rest assured. Many thousands of have now successfully come off and are living proof that it can be done. And now there’s a whole variety of withdrawal support resources available. (see below)

What is EFT?

Cyclobenzaprine hydrochloride has the most recent and largest clinical trials demonstrating its benefit, but carisoprodol, diazepam and metaxalone also appear to be effective.

In 1989 researches compared the effectiveness of cyclobenzaprine (Flexeril) alone with diflunisal (Dolobid), placebo, and a combination of cyclobenzaprine and diflunisal in the treatment of acute low back pain and spasm. During the ten-day study period, the combined treatment group demonstrated significantly superior improvements in global ratings on day four, but not on day two or seven. This study suggested some effectiveness of combined analgesic and muscle relaxant therapy when utilized early in the initial week of pain onset.

Another study compared the effects of combined cyclobenzaprine and naproxen (Naprosyn) with naproxen alone and also found combination therapy to be superior in reducing tenderness, spasm, and range of motion in patients with low back pain and spasm.

Cyclobenzaprine and carisoprodol were compared in the treatment of patients with acute thoracolumbar pain and spasm rated moderate to severe and of no longer than seven days duration. Both drugs were effective, without significant differences between the treatment groups. Significant improvements were noted in physician rated mobility and in patients’ visual analogue scores on follow up days four and eight. While 60% of patients experienced side effects in the form of drowsiness or fatigue, these differences were not significantly different between treatment groups and only eight percent of patients from each group discontinued treatment.

In an attempt to determine the mechanism of action of carisoprodol in the treatment of low back pain, a study was carried out comparing its effectiveness to that of a sedative medication butabarbital, and a placebo. Carisoprodol was significantly more effective in providing both pain relief and improvements in range of motion. The results of this study suggest that the effects of carisoprodol are not secondary to its sedative effects alone. In addition to the skeletal muscle-relaxing effects, carisoprodol also produces weak anticholinergic, antipyretic, and analgesic effects.

In an earlier study, diazepam (Valium) was found to offer no significant subjective or objective benefit, when compared to placebo, in patients treated for low back pain. Carisoprodol (Soma) was found to be superior to diazepam in the treatment of patients with "at least moderately severe" low back pain and spasm of no longer than seven days duration. In this study, the overall incidence of adverse reactions was higher in the diazepam treated group but was not of statistical significance.
